Please review the liability and photo release below. This form is needed for you to participate in General Events that are run or hosted by Royal View Young Adult Ministry. These events include midweek programs and on- and off-site events both in the community and at Royal View under Young Adult Ministry, but are not limited to these specific events. This information will be used to to ensure that you are able to have a safe time while with us.
Medical: Precautions are taken for the safety and health of attendees, but in the event of accident or sickness, Royal View Church, its staff, and volunteers are hereby released from liability. If you require special medication, x-rays, surgery or other medical treatment, effort will be exercised to contact the emergency contact listed on this form. However, if notification is not possible, by signing below you hereby authorize Royal View Church, staff and volunteers to seek medical attention on your behalf.
Promotional: Photographs, video, and audio are taken weekly at our programs. These media tools are for promotional and educational use only and to share our events with Church and Community. By signing below you authorize Royal View Church, staff and volunteers to photograph, video, or record you for the purpose of advertising and promotions.
Safety Guidelines: By signing below you agree that you will make every effort to abide by the safety guidelines of the programs, and events at the Royal View Church. Should you refuse to abide by the guidelines or disrupt programs or other attendees, you will be removed from the program/activity until you are ready and able to return. If you repeatedly choose not to abide by the safety guidelines, you will be sent home at your own expense and asked not to return for a short period of predetermined time.
Transportation: By signing below I give permission to ride the bus when necessary for special event or weekly pickups and drop-offs. As well to drive in a car with leader supervision for special trips and events. I understand that organizing a ride with another attendee is not liable under Royal View Church.
Purpose and Extent: Royal View Church is collecting and retaining this personal information for the purpose of enrolling you in our program, to assign attendees to the appropriate groups, to develop and nurture ongoing relationship with attendees and to inform you of program updates and upcoming opportunities at our church. This information will be maintained permanently as it is a requirement of our insurance company and legal counsel. If you wish Royal View Church to limit the information collected, or not to view your information, please contact us.
